After you book the New Client Initial Consult, follow the steps below to prepare:
Step 1: Client Intake Form
This will be sent to you automatically via email once you have booked your consultation.
Fill in your information and submit your form.
Step 2: Biopsy and/or Surgical Pathology Report
Step 3: Current routine blood test results
If you do not have current lab test results (less than 8 weeks) for the baseline and need to order blood tests, follow the instructions below on "Ordering blood tests”
Step 4: Latest imaging reports
Radiology reports from PET/CT, CT, MRI, Whole Body Bone Scan, Ultrasound, Mammogram, or plain x-rays, if any.
Step 5: Submit the documents for review

CA 125 (for Ovarian Cancer)
Thyroglobulin (for post-thyroidectomy Papillary Thyroid Cancer)
S100B (for metastatic Melanoma)
